Path: Admin > Form Attributes > Service Request Form / Identity Tab
The Service Request Form / Identity Tab is used to establish basic information about your new Request window. The only required fields are the Name (your identifying name/code) and Form Type.
If you allow access to the window without using a login, check the Use Without Login box.
Once you have established the Identity Tab settings, click the Repair Center Tab to mark one or more Repair Centers linked to your Request Form.
You can only see previously created forms that are linked to your assigned Repair Centers.
Elective Fields
These are the steps taken to create a record and start the process.
- Open the Service Request Form window. If you cannot open the window, see the Note in Service Request Form Overview.
- Click Add on the WebTMA toolbar.
- Type a Name.
- Select a Form Type and complete any fields on the Identity Tab as needed.
- Click the Repair Center Tab.
- Select the applicable Repair Centers to which the form will be linked.
- Click the Layout Tab.
