Four other Tabs are available on the Service Request Form page. This gives you the option to create confirmation status and add other messages in response to the request. These are not required.
The layout and buttons are the same as the Forms Submission Tab.
Confirmation Tab
Path: Admin > Form Attributes > Service Request Portal – Forms / Confirmation Tab
To give your requestors a confirmation that their request was received, you can use the Confirmation Tab to provide that information. The number of options on the Block Palette is limited. Of note is the Reload Button, which Requestors can use to return to the initial Service Request window and submit another request.
When requestors click Submit on their request, the page automatically refreshes to display the Confirmation text you add to this tab. In addition, a hard-wired email message is sent with a link that requestors can use later to check the status of their request. As a reminder, the Submission must include a REQUESTOR EMAIL block for messages to be sent.
Status Tab

When requestors want to check the status of their request, you can set up the Status Tab to provide the information. Like the Confirmation Tab, the blocks are limited on the Status Tab. The Reload Button is also available on this Tab.
Submission Error Tab
Path: Admin > Form Attributes > Service Request Portal – Forms / Submission Error Tab
If a request is not completed, you can use the Submission Error Tab to provide them with information about the problem and include helpful instructions and links.
Status - Not Found Tab
Path: Admin > Form Attributes > Service Request Portal – Forms / Status - Not Found Tab
Like the Submission Error Tab, you can add information to the Status - Not Found Tab to assist users who search for a request status.
